Requesting Your Health Records
Clients have the right to request access to their health care records.
To request your records:
Submit a written request by email, or
Contact our office directly using the information listed on our Contact page
Requests will be processed in accordance with applicable state and federal laws, including HIPAA and Texas Health & Safety Code requirements.

If you have questions or concerns about professional conduct, you may contact the appropriate licensing or disciplinary authority.

Filing a Consumer Complaint
Consumers have the right to file a complaint regarding health care services or privacy practices.
You may:
Submit a complaint directly to the Texas Behavioral Health Executive Council using the contact information above, or
File a complaint regarding health information privacy with the U.S. Department of Health & Human Services, Office for Civil Rights (OCR)
