Requesting Your Health Records

Clients have the right to request access to their health care records.

To request your records:

  • Submit a written request by email, or

  • Contact our office directly using the information listed on our Contact page

Requests will be processed in accordance with applicable state and federal laws, including HIPAA and Texas Health & Safety Code requirements.

Licensing & Disciplinary Information

If you have questions or concerns about professional conduct, you may contact the appropriate licensing or disciplinary authority.

Filing a Consumer Complaint

Consumers have the right to file a complaint regarding health care services or privacy practices.

You may:

  • Submit a complaint directly to the Texas Behavioral Health Executive Council using the contact information above, or

  • File a complaint regarding health information privacy with the U.S. Department of Health & Human Services, Office for Civil Rights (OCR)
